For Birth and Death records after 1908 contact the local health department. For births/deaths occurring in Miami County except the city of Piqua, contact the Miami County Health Department at (937) 440-5450.  For births/deaths occurring in the City of Piqua, contact the Piqua Health Department at (937) 778-2060.
Cost for certified copies of marriage license is $2.00. Costs for certified birth/death records through Probate Court are $3.00. Requests by mail should include a self-addressed stamped envelope.
While most Probate Court records are open to the public, Section 149-43 of the Ohio Revised Code closes records pertaining to mental and medical examination or adoption.
